I find it extremely curious at times meet with companies that come to me (BTW for any crap that could have been solved with a call or better yet, an exchange of emails) and it appears the CEO, CTO and almost to the CSI and when minimally one scratch in the company they are, nobody else.
Not only do I believe that these structures are surreal type: All heads, not a single Indian, but also always think "fuck, if you are 3 and 3 are here, who the hell is working?".
In matters related to technology, I think, is even desirable that structures can and should be above all three things: horizontal, dynamic, and scalable.
Many examples of poor sustainability in recent times that have made when, for example in the case of online media, advertising low, has drama. The trouble is that sometimes, despite knowing yourself well the theory, self falls on those mistakes, and yes, my me-partner and I happened several times that. Big mistake.
The Indians are necessary in any structure, and on the internet tend to make the whole Christ "head" of something, almost no Indian. Directors of Communication in which they are the only people in the department, as CTOs, CEOs etc themselves. It is a fake that looks great on business cards, but occasionally rubs surrealism.
I bet conceptually lateral structures that the individual freedom and a certain freedom vertical, which reports to a single person and are almost completely horizontal, with only two levels and are far from the classic pyramid scheme, which sometimes, in the case large companies, the information reaches the top, where decisions are capitalized, always when it's late and when the problem is already on the table.

In the post I agree with you, even some people are once CEO, Managers in another and, when given the need, Counselor at another, and even when it is the case, Founders, Lecturers, Partners and Investors all in one.
Are the side effects of unbridled passion for labels and respect based on the hierarchy ... pity anyone remember the # 50 of Cluetrain Manifesto:

I personally am a fan of the concept of 'internal customer'. Everyone, even CEOs have to prepare and sell well domestic product (such a good sale or a large merger in the case of the CEO) to an internal client (as some workers who have chosen this project out of better paying jobs and safer, or investors themselves).
So nobody is resting on its laurels, and there is more flexibility to negotiate within the team itself, and know better all processes.
That is another reason why it is difficult to get investors because such practices are losing confidence.
Surely these "companies" are those that "sell smoke" and then break like many already have broken recently.
